What happens after we lose citizenship

  • Complete the Form N-565 Application for Replacement Naturalization/Citizenship Document. Review the form instructions for directions on completing the Form N-565.

  • Submit the Filing Fee(s).  Include the appropriate filing fee with the Form N-565 if filing by paper, or paying online. 
     
  • Photographs.  You must have two passport-style photos taken and submit them with your application. If you file online, you must submit the same photos that were uploaded with the application. Please refer to the Form N-565 instructions for more information on the specific requirements for submission of your photos.
     
  • Submit Evidence. Include all required initial evidence and supporting documentation for the Form N-565.  Review the Form N-565 instructions for additional requirements. Evidence can be submitted online.
     
  • Submit Documents to Reflect a Legal Name Change. If you have had a name change, attach the original document and a copy of the marriage certificate, divorce decree, annulment decree, or court order showing the name change.

  • Submit Original Document.  You may be required to submit original documents when you file, or after filing online if you are applying for a replacement for one of these reasons:

    • Your document has been mutilated,  
    • has an error caused by USCIS,
    • your name legally changed,
    • your date of birth legally changed,
    • gender legally changed
       
  • Sign and File the Form N-565. Sign and submit the application online, or file at the correct filing location according to form instructions.
